This form is for a 4 person squad who will be shooting both the smallbore and air rifle event.
This form should be filled out by the coach or leader of the team. This will be the basis for your start list that can be changed up to 30 minutes prior to call to the line.
Once the form is completed you will be directed to the online store where the appropriate item may be added to the check out cart and paid for with credit card. If payment by check is preferred please make the check payable to The American Smallbore Shooting Association. Mail it PO BOX 855 Latrobe PA. 15650. Please add a note in the comments section of the form if you are paying by check.
